The Doctors\u2019 Association UK (DAUK) has warned that doctors are being side-lined and professional responsibilities blurred.\r\n\r\nDAUK fears a new competencies framework represents the creeping substitution of doctors with other healthcare professionals (HCPs).\r\n\r\nThe recently published National Capabilities Framework for Professionals who care for Children and Young People with Asthma<\/a> sets out a five-tier structure of competencies.<\/p>\n\n\n\n

Professional responsibilities<\/h3>\n\n\n\n

At the lowest tier, GP receptionists, childcare staff, and teachers will provide \u2018basic asthma awareness\u2019 and signposting.\r\n\r\nGPs, resident doctors, and emergency department doctors are listed in tier three out of the five.\r\n\r\nThey can \u2018diagnose, assess, and manage acute and chronic asthma\u2019 and \u2018address factors that contribute to poor control\u2019, the document states.\r\n\r\nAt the higher levels of four and five, paediatricians with special interest, advanced nurse practitioners, tertiary paediatrician, and allied health professional (AHP) member of the asthma multidisciplinary team (MDT) are listed.<\/p>\n\n\n\n

Replacing doctors<\/h3>\n\n\n\n

Dr Neena Jha, a member of DAUK\u2019s GP committee and a GP in Hertfordshire, said the framework represented the direction of travel under the Government\u2019s 10-year NHS plan.\r\n\r\nPosting on X<\/a>, formerly Twitter, Dr Jha said the plan was \u2018dismantling the practice of medicine\u2019 and would \u2018replace doctors with other HCP \u201cto deliver task-based care\u201d\u2019.\r\n\r\nIn a second post on X<\/a>, she said: \u201cIn short, the Government\u2019s 10 year NHS plan is to replace doctors with other HCP at scale.\u201d\r\n\r\nDr Alison George, who is also a member of the DAUK GP committee, said the document represented \u2018taskification and tiered care in all its glory\u2019 in a post on X<\/a>.<\/p>\n\n\n\n

Private healthcare<\/h3>\n\n\n\n

She added in a separate post on the social media website<\/a>: \u201cPrivate healthcare providers are circling with excitement.\r\n\r\n\u201cMany are already embedded in the NHS, and Department of Health and NHS England\u2019s doctor replacement policy opens up opportunities for more.\r\n\r\n\u201cVery soon it will be impossible to guarantee doctor-led care unless you pay\/have insurance.\u201d\r\n\r\nThe framework was initially highlighted on X<\/a> by acute physician Dr Louella Vaughan.\r\n\r\nDr Vaughan said: \u201cHere is an example of what is coming.<\/p>\n\n\n\n
